Preserve the original record
Keep the initial brief, approved artwork, accepted assembly, supplied version and relevant evidence. Describe the reported issue and observations without assigning unsupported blame. A remake should not erase the earlier decision or convert an unresolved defect into a known cause. Contractual responsibility remains with the actual parties and applicable review.
Define the corrective question
State what the remake must change and which criteria remain valid. Identify material, print, finish, dimensions, fixing or message changes separately. An unchanged image does not approve a changed physical construction, and a new production run is not automatically equivalent service.
Record additional activity honestly

If a comparison or project report includes this activity, preserve the boundary and version it accordingly. Do not hide an extra run inside an optimistic planned figure or invent an avoided-impact offset.

It describes the rejected face for an actual receiver enquiry rather than assuming that remaking the item proves the first one was recycled.
Close the actual outcome
Record the corrected version supplied, responsible acceptance, original-item destination and remaining actions. Keep a factual correction trail for public statements affected by the change.
