Work through the decision
Identify the answer version, exact disputed wording and allowed task context. Explain the actual source mismatch, missing evidence, calculation issue or permission failure. Include only relevant redacted evidence. Record the correction and affected outputs, preserving the original history so the failure can inform evaluation rather than being hidden.
A fictional worked example
In a fictional answer, a generic receiver profile is mistaken for batch acceptance. The learner reports the exact statement and the absent receiver response. The corrected answer returns to an acceptance request and identifies any draft derivative that repeated the unsupported outcome.
Evidence and authority limit
Reporting an error does not prove every other statement is correct or authorise exposure of private records.
